The Sacramento County Community Health Improvement Plan (CHIP) works to build healthier communities in Sacramento County. The CHIP is an ongoing, community-led process of defining health needs, setting priorities, and creating a plan to meet them. Based on data compiled through a community health assessment in 2023, the three CHIP priorities are: mental health, housing insecurity, and food access.
Geographically, CHIP activities will focus on the census tract with the lowest Healthy People Index (HPI) score, census tract 44.01. This area is partly in the City of Sacramento and partly in unincorporated Sacramento County.
Public Health Advocates is proud to be a key partner in this work. The County of Sacramento has approved Public Health Advocates to manage the $1.3 M funding for CHIP activities until May 2026.
In the summer of 2024, the CHIP steering committee is seeking an agency that works in Sacramento communities and in partnership with other organizations to oversee and train 30 youth and adult community members on community organizing and policy advocacy to lead community outreach and training for the CHIP.
